This Campaign is initialed by Magnum Group Employees to help bring awareness of the importance of eye care amongst young children. This is the second year that the campaign is being organized. We engage professionals to carry out the eye-screening and our Teams will provide the needy students/children the corrective spectacles.

I, the undersigned, certify that all the information on this form is correct to the best of my Knowledge. I give the permission for the above child to participate in ICSCN Kids Fun Workshop at CityOne Megamall. The Organizer reserves the right to use the participants' name, photographs or video for the purpose of publicity, advertising, marketing and distribution to the sponsors. There will be no fee refund if the event is cancel due to occurrence of incidents that are beyond the control of the Organizer. The Workshop will expect to start on time. Please arrive 10 minutes before the scheduled start time. Children will involve in mild cooking and scissor cutting. Workshop will be conduct in safe environment but event organizer will not held the responsible for any unexpected injuries during the workshop. I confirm that I have read and accepted the conditions of entry to "I can see clearly now" Charity Carnival and the rules, regulations for this event.
